Urgent surgical care and specialized management are required for:
Deep, open sores on the bottom of the foot or toes that do not heal with standard bandaging.
Redness, extreme swelling, and foul-smelling drainage indicating a deep tissue or bone infection (osteomyelitis).
Blackened, dead tissue caused by a severe lack of blood flow that must be surgically removed to stop the spread.

Surgical cleaning (debridement) of the wound is performed. The patient receives strong IV antibiotics to rapidly bring the infection under control.
